Preparing a Label Package

The label package is made up of a paper label, which is inserted into a plastic sleeve. The paper label must have the following words which can be clearly visible to a person when purchasing the product “IMPORTANT – FOLLOW THESE DIRECTIONS”. The label must also contain the new instructions for use (PDF document PDF - 58KB). The Label Package must be securely affixed to the container.

Keeping records of supply

Registrants and retailers must keep a record of the products supplied with the Label Package affixed. The particulars in the record include the name of the product supplied, the quantity supplied, the date of supply and the name and address of the purchaser. The form of record is not stipulated however the following format in a spreadsheet is suggested. A registrant or retailer may be required to submit records to the APVMA:

Registrants will also be required to provide objective evidence to the APVMA that they have provided Product Alert Notices to retailers for distribution to end-users and sufficient Label Packages to re-label quarantined product at retail premises. The number of Product Alert Notices can be reported as one figure. For Label Packages keeping a record as set out below will assist in satisfying this requirement. Combining both records into one will also be acceptable.

Retailers must keep a record of the Product Alert Notices distributed to customers who have purchased product in the last 12 months. The particulars in the record include the date of supply and the name and address of the customer. The form of record is not stipulated however the following format in a spreadsheet suggested. A retailer may be required to submit records to the APVMA:
